About The Position

Suncrest Hospice is seeking a Registered Nurse (RN) to provide care and services for a group of patients on a PRN (as needed) basis. The RN will visit patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities. This role is a key member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources.

Requirements

  • Current Registered Nurse (RN) licensure in good standing in the state of Ohio.
  • Ability to work collaboratively within a team environment with nurses, social workers, chaplains, volunteers, and aide leadership.
  • Ability to work independently and manage daily activities including phone, email, and written communication.
  • Ability to attend meetings with a variety of healthcare professionals.
  • Understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership.
  • Work closely with patients on a 1-1 level but get strong support from a patient care team.
  • Can work independently and on your own on a day-to-day basis with activities which include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns)
  • Ability to learn an electronic medical records system.
