Hospice Registered Nurse - Case Manager - full time days
About The Position
Registered Nurse (RN) full time opening in our hospice division for patients residing in their own home or facilities/nursing homes we serve for Home Health & Hospice Care. Offering flexible schedule, ability to work autonomously once trained, exceptional team support with a highly skilled Manager and Team Lead for hospice / end of life care. 40 hours per week - Mon to Fri with weekend and holiday rotation. Service Areas: Patient homes in Southern New Hampshire greater Merrimack, Nashua, Manchester communities. We are now utilizing EPIC for our electronic medical records!
Requirements
- Graduate of an accredited school of nursing.
- Associate degree in nursing.
- Registered as RN with New Hampshire’s Board of Nursing.
- A minimum of one year recent acute care experience.
- Ability to exercise initiative and independent judgment.
- EPIC experience preferred, willing to train!
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
Nice To Haves
- Bachelor degree in nursing.
- Community Health/Education experience.
- Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
- EMR software proficiency.
Responsibilities
- Completes an initial home health assessment of patient and family to determine hospice care needs.
- Provides a complete physical assessment and history of current and previous illness(es).
- Utilizes the nursing process in planning, providing and evaluating skilled needs.
- Completes a comprehensive patient assessment.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
- Includes the patient and family on the planning and revising process.
