About The Position

As a Hospice Registered Nurse Case Manager, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, and educating patients and families. Trinity Health At Home – Grand Haven is a trusted member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities across 11 states. As a faith-based, not-for-profit agency, we are committed to caring for patients and families in the place they feel most comfortable—at home. Our services include skilled nursing, physical, occupational and speech therapy and medical social work.

Requirements

• Active RN license in Michigan
• 1+ years of clinical nursing experience (Hospice preferred)
• Strong communication, assessment, and organizational skills
• Compassionate, dependable, and mission-minded
• Valid driver’s license and reliable transportation required
