Requirements

  • Graduate of an accredited nursing program and active RN license in California. License is active and in good standing.
  • Minimum of least two (2) years of experience as a hospice RN and at least one (1) year of supervisory experience.
  • CPR certification, reliable transportation, valid auto insurance and driver's license
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion

Responsibilities

  • Serves as the primary clinical RN for assigned night shifts, providing after-hours coverage and clinical oversight.
  • Answers and triages after-hours calls from patients, families, facilities, and staff using sound clinical judgment.
  • Documents all interactions, assessments, and interventions timely and accurately in the electronic medical record (EMR).
  • Provides nursing visits as clinically indicated, including symptom management, crisis intervention, and Start of Care (SOC) visits when assigned.
  • Supports workload offloading from daytime staff by completing urgent or high-acuity visits as needed.
  • Provides clinical guidance and supervision to professional and paraprofessional staff during the night shift.
  • Performs assigned quality assurance checks and identifies documentation or compliance gaps.
