Requirements

• RN with current RN licensure in the states of Kentucky and/or Indiana as required by designated position, or the ability to obtain required license upon hire.
• 1 year of nursing experience.
• High level of computer skills to navigate multiple screens to complete admission.
• Proven time management skills and ability to toggle between multiple tasks.
• Ability to work autonomously and recognize when other disciplines are needed.
• Must have a valid driver’s license.

Nice To Haves

• ER, ICU, Home Health experience a plus.
• Knowledge of Medicare/Medicaid reimbursement helpful.
• Knowledge of palliative care helpful.
• Use of technology-based tools, including electronic medical record, strongly preferred.

Responsibilities

• Responding to new referrals to identify needs, offer hospice solutions, and admit medically eligible patients into Hosparus Health care.
• Coordinating care and services.
• Educating referral sources, patients, and their families about the hospice benefit and options available.
• Providing premier customer service.
