INTEGRIS Health is seeking a compassionate and collaborative Hospice physician to support Metro Oklahoma City Hospice Services. The successful candidate will join our engaged interdisciplinary team and exhibit expertise in the provision of high-quality hospice care. Requirements

Oklahoma MD or DO license (presently have)
BC/BE in Internal Medicine, Family Medicine, Geriatrics, or related specialty

Nice To Haves

Fellowship in Hospice & Palliative Care preferred though not required

Responsibilities

Physician oversight for Medicare and Medicaid hospice programs, ensuring that medical care is provided in accordance with federal Conditions of Participation (CoPs) and quality standards
Provide hospice and palliative care consultations for inpatient and home hospice settings
Engagement with Quality metrics including Hospice Determinations, Diagnosis Relatedness, CTI completions, KPIs
Lead discussions regarding goals of care, advance care planning and symptom management
Inpatient/outpatient rounding and oversight, medication orders, admissions, IDG coverage weekly, and call coverage (added call compensation)
Participate in care transitions and coordination across the continuum of care
Collaborate closely with interdisciplinary teams including nursing, social work, chaplaincy, pharmacy and case management
Shared call coverage with supportive physician team
Rotations at Hospice House in NW OKC – 12 bed unit (shared among several physicians)
