Hospice PCS
About The Position
As a Hospice PCS, you will play a vital role in enhancing quality of life for patients and supporting families during some of their most important moments. Working alongside a dedicated interdisciplinary team, you'll deliver compassionate care that brings comfort, dignity, and peace. This is a field-based role requiring travel to patient homes and care facilities throughout the Zanesville service region. This is a FT position, 4-10hr shift of every Wednesday-Saturday, 8am-6pm; This is a driving position seeing patients in both facilities and homes within the greater Dayton/Trotwood area.
Requirements
- High School Diploma or GED
- Proficient computer skills with the ability to accurately document patient care and services in electronic medical records.
- Valid Ohio driver’s license with a safe driving record and proof of auto insurance.
- Ability to pass a criminal background check and 10 panel drug screening.
Nice To Haves
- Certified Nurse Aide (preferred, but will train internally)
- Certified Hospice and Palliative Nurse Aide (CHPNA) certification preferred. Ohio's Hospice supports professional growth and offers opportunities for eligible employees to obtain certification.
