The needs of our patients are our utmost priority. As a Patient Care Manager, you are a member of the dedicated clinical management team that is responsible for providing management and leadership of the respective clinical hospice team. The team consists of Registered Nurses (RN) and Certified Hospice Aides who provide hospice services in a designated territory. This position is responsible for leading interdisciplinary group meetings and participate in the development of the nursing team. This position is primarily in the office, with expectations of being in the field when needed. Candidates should have leadership skills with experience and knowledgeable in hospice policies and procedures. This is a salaried non-exempt position working Monday - Friday, 8:00am-5:00pm. The clinical management team rotates On Call AOC responsibilities during the week.
