Silverado Hospice San Diego is seeking a compassionate and experienced Hospice Patient Care Manager / Clinical Manager (RN) to lead clinical operations and support interdisciplinary teams. Requirements
- Registered Nurse with a valid state license.
- Minimum of 1 year of experience in healthcare management, ideally in hospice.
- Valid driver's license and auto insurance.
- Reliable transportation and willingness to drive to patient locations.
- Ability to lift up to 50 pounds.
Responsibilities
- Oversee clinical operations and support interdisciplinary teams.
- Serve as a liaison between the Director of Patient Care Services, clinical staff, patients, families, and the community.
- Oversee scheduling, workflow, and compliance for clinical associates.
- Provide routine patient care in the absence of other clinical staff members.
- Ensure adherence to hospice policies, state and federal regulations, and Silverado standards.
- Support quality improvement initiatives and interdisciplinary team collaboration.
