About The Position

Patient Care Coordinators oversees the day-to-day clinical operations of the Hospice department. Monitors quality of care and leads and participates with team members to develop, implement and communicate process improvements.

Responsibilities

  • Collects pertinent data and information relative to the patient's health or the situation.
  • Recognizes normal and abnormal findings when gathering data.
  • Assigns staff to ensure appropriate caseload, and coordinates daily/weekly scheduling.
  • Assist in the development and implementation of performance standards, policies and procedures which align with regulatory and accreditation requirements and guidelines.
  • Assures that clinical records, both paper and electronic, meet regulatory and accreditation requirements, and that all components are completed, received, and entered in a timely manner.
  • Assures timely response to and intake of referrals and coordinates admission process.
  • Attends meetings, in-services, and continuing education.
  • Plans and evaluates care in collaboration with appropriate disciplines.
  • Analyzes assessment data to determine actual or potential diagnosis, problems, and issues.
  • Evaluates progress toward attainment of goals and outcomes.
  • Reports data and outcomes to others as appropriate.
  • Implements care or work plan in alignment with the plan and approved safety, infection control, and department/organization standards.
  • In collaboration with leaders, actively pursues required knowledge and skills through orientation activities specific to the position, reading current literature and seeking new learning opportunities.
  • Leads interdisciplinary team meetings, and guides and fosters a productive discussion environment.
  • Manages quality of care through supervision of field staff through direct observations, as well as soliciting patient/caregiver feedback through patient rounding.
  • Performs performance appraisals and annual evaluations.
  • Monitors electronic systems for errors and corrects these as they are found.
  • Monitors quality and compliance through performance of assigned audits.
  • Identifies expected outcomes for a plan individualized to the patient or situations.
  • Oversees the day-to-day clinical operations of the Hospice Department.
  • Performs visits as needed for informational, admission, or patient care service.
  • Develops a plan that prescribes strategies to attain expected, measurable outcomes.
  • Establishes effective working relationships with peers, physicians, and other members of the health care / work team.
  • Identifies and confers with appropriate resources regarding patient / work decisions.
  • Supervises field staff and provides orientation, training, education, and guidance.
  • Oversees and approves payroll of assigned staff.
  • Employs teaching strategies to promote health and a safe environment.
