About The Position

Concho Hearts Hospice PRN On-Call RN 7 days on / 7 days off On-Call cycle Area: Pilot Point / Denton / Sherman area Concho Hearts Hospice's Mission Statement is to provide compassionate care wherever you call home. Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and Doctors provides symptom management and support for patients and their families as they cope with an end-of-life illness. Job Summary: This is an on-call-only position; it will be a cycle of 7 days on call and 7 days off call. It is a full-time position. Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency; and assume the responsibility for coordination of care.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current license as a registered nurse (RN) and/or by the Board of Nurse Licensure Compact (NLC), current Driver's License.
Two years of experience as a Registered Nurse in a clinical care setting preferred; hospice preferred.
Nursing skills as defined by generally accepted standards of practice.
Good interpersonal skills.
Specialized skills preferred in palliation and end-of-life.
Proof of current CPR.
Must read, write, and comprehend English.
Reliable transportation.
Valid and current auto liability insurance.

Nice To Haves

Hospice experience preferred.

Responsibilities

Preparing and adjusting patient care plans as needed.
Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
Maintaining and updating medical charts, patient files, and health records.
