About The Position

LDV Hospice is seeking a compassionate and experienced On-Call Registered Nurse (RN) to provide after-hours nursing care and support to hospice patients throughout our service area. This position plays a vital role in responding to urgent patient needs, providing symptom management, conducting nursing assessments, and offering guidance and reassurance to patients and families outside of normal business hours. The ideal candidate is confident working independently, exercises sound clinical judgment, and remains calm and compassionate in time-sensitive situations. Requirements

  • Graduate of an accredited school of nursing.
  • Current and unrestricted Registered Nurse (RN) license in the State of New Mexico.
  • Minimum of one year of nursing experience.
  • Current driver's license, reliable transportation, and proof of automobile insurance.
  • Excellent assessment, critical thinking, and communication skills.
  • Ability to work independently and make sound clinical decisions in the field.

Nice To Haves

  • Experience in hospice, home health, palliative care, or emergency nursing.
  • Experience with electronic health record (EHR) systems.
  • Knowledge of symptom management and end-of-life care.

Responsibilities

  • Respond promptly to after-hours patient calls and provide skilled nursing care in the home.
  • Assess patient conditions and provide appropriate symptom management interventions.
  • Coordinate care with physicians, the hospice interdisciplinary team, and other healthcare providers.
  • Educate patients and families on medications, disease progression, and comfort-focused care.
  • Perform emergency nursing visits as needed to maintain patient comfort and safety.
  • Accurately document all patient assessments, interventions, and communications in the electronic medical record.
  • Provide emotional support and compassionate guidance to patients and families during difficult moments.
  • Communicate effectively with the clinical team to ensure seamless continuity of care.
