Requirements
- Graduate of an accredited nursing program with an active RN license in Virginia
- CPR Certification
- Minimum of two (2) years of nursing experience
- Knowledge of state and federal home health/hospice regulations
- Excellent clinical, communication, and critical thinking skills
- Valid driver’s license and reliable transportation

- Prior hospice and/or on-call experience
Responsibilities
- Provide after-hours and weekend on-call nursing support to hospice patients and families
- Respond to urgent patient needs, including symptom management, changes in condition, and crisis intervention
- Complete initial and ongoing comprehensive assessments, including physical and psychosocial status
- Administer medications and treatments in accordance with physician orders and nursing standards
- Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary group (IDG), physician, and patient/family
- Coordinate care across the IDG and external providers to ensure continuity and quality of care
- Evaluate patient needs and facilitate appropriate levels of care (e.g., inpatient, respite, continuous care) when indicated
- Provide education and emotional support to patients and families regarding care plans, medications, and disease progression
- Ensure availability of medical equipment, medications, and necessary supplies
- Supervise and direct care provided by LPNs, hospice aides, and other support staff
- Participate in interdisciplinary team meetings and contribute to care planning
- Maintain accurate, timely, and compliant documentation of all patient interactions and services provided
- Support quality assessment and performance improvement (QAPI) initiatives
