About The Position
Hospice On-Call Registered Nurse – PRN Pottsville, PA Step Into a Fulfilling Career with Advantage! We're currently seeking a passionate and skilled Registered Nurse (RN) to join our ever-expanding Hospice team in the Pottsville, PA and surrounding areas. This role offers the chance to directly provide patient-centered care in the home that focuses on comfort, dignity, and support during life's most challenging moments, truly making a difference in the lives of our patients.
Requirements
- An active RN license in the state in which practicing
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Compassionate and empathetic approach to patient care
- Ability to work independently and as part of a team
- Knowledge of hospice regulations and standards of care
- Reliable transportation and willingness to travel
Nice To Haves
- Minimum of 1 year of experience preferred
Responsibilities
- Conducting Start of Care comprehensive assessments
- Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
- Preparing accurate, orderly, and timely documentation
- Coordinating care with patient and provide proper reports to Case Manager
- Provides health care instructions to the patient/family as appropriate per assessment and plan
