About The Position
Samaritan is seeking a compassionate and dedicated Hospice Nurse to oversee the quality, expert care of assigned hospice patients. This role is crucial in providing comfort and relief of symptoms, establishing individualized plans of care, and educating patients and families. The Hospice Nurse will also obtain physician orders for symptom management, triage and arrange home visits efficiently, and collaborate with other members of the interdisciplinary team, including social workers.
Requirements
- Minimum of two years of nursing experience.
- NJ RN license required

- BSN preferred
- Bilingual is a plus!
Responsibilities
- Perform patient assessments for comfort and relief of symptoms.
- Establish and implement an individualized plan of care for each patient.
- Assess the need for patient and family education and provide education as needed.
- Obtain appropriate physician orders to achieve symptom management.
- Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
- Make referrals to additional IDT members and collaborate with social workers related to their involvement.
