The Hospice Nurse identifies patient/family care needs and provides nursing and supportive care for terminally ill patients and their families. This role focuses on delivering quality nursing care to patients with life-limiting illnesses, ensuring the highest level of care in collaboration with the primary physician through the nursing process. Key responsibilities include participating in an interdisciplinary team to develop and implement individualized care plans, consulting with attending physicians, and evaluating patient/family responses to interventions. A significant aspect of the position involves providing patient and family education to foster skills for quality care, promote independence, and facilitate living fully until death, including teaching infection control protocols. The nurse is also responsible for accurate and timely documentation, participation in Quality Improvement Programs, and providing direction and supervision to Patient Care Partners, Unit Receptionists, and volunteers.
