This role involves providing skilled nursing care to patients at the end of life, conducting visits in both facility and home settings. The nurse will provide on-call services after hours and complete scheduled skilled visits. Responsibilities include responding to patient and family calls regarding symptoms, changes in condition, or emergencies, and conducting home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis. The nurse will also assist the RN Case Manager (RNCM) in carrying out the plan of care, performing specialized procedures as directed, and participating in interdisciplinary group (IDG) collaboration to ensure the patient plan of care is current. Additionally, the role involves reinforcing patient and family education regarding end-of-life care, assuming responsibility for personal and vocational growth, keeping the RNCM informed of changes, participating in self-development through further education and professional activities, and assisting with emergent visits including continuous care (CC) and companion care as needed. Other duties as assigned are also part of the role.
