About The Position

This role involves providing skilled nursing care to patients at the end of life, conducting visits in facilities and/or homes. The Hospice Nurse will also provide on-call services after hours and complete scheduled skilled visits. Responsibilities include responding to patient and family calls regarding concerns about symptoms, changes in conditions, or emergencies. The nurse will conduct home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis. This position assists the RN Case Manager (RNCM) in carrying out the plan of care and performing specialized procedures as directed. The nurse will actively participate in interdisciplinary group (IDG) collaboration to ensure the patient plan of care is current and issues are addressed. Additionally, the role involves reinforcing patient and family education regarding end-of-life care, assuming responsibility for personal and vocational growth, and keeping the RNCM informed of changes. Participation in self-development through further education, workshops, conferences, and professional organizations is encouraged. The nurse will also assist with emergent visits including continuous care (CC) and companion care as needed, and perform other duties as assigned.

Requirements

  • Licensed RN in the state of practice
  • A minimum of 1 year of Clinical experience
  • Valid Driver's License and Auto Insurance Coverage
  • Reliable transportation, willingness to travel
  • On-call/ skilled nursing experience required

Responsibilities

  • Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
  • Provide on-call services after hours, and complete scheduled skilled visits.
  • Responding to phone calls from patients and their families regarding concerns about symptoms, changes in conditions, or emergencies.
  • Conducting home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis.
  • Assist RNCM in carrying out the plan of care.
  • Assist the Nursing team in performing specialized procedures( as defined in the state nurse practice act) as directed by the RNCM under the physician's orders.
  • Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
  • Assist in the reinforcement of patient and family education regarding end-of-life care.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
  • Keep the RNCM informed of changes noted during the process.
  • Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading.
  • Assist with emergent visits including continuous care (CC) and companion care as needed.
  • All other duties as assigned
