About The Position

The Hospice Nurse will provide quality care to patients wherever they consider home, including personal residences, assisted living facilities, and skilled nursing facilities. This role involves training caregivers on patient care, including wound care, medication administration, personal care, and end-of-life expectations. The nurse will spend significant time driving between patients, documenting care, and coordinating with community medical providers, hospice providers, and other disciplines. Patients are assigned based on team needs, acuity, and geography. Documentation is done in real-time at the patient's home, and medications/supplies are ordered online. The nurse will self-schedule based on patient needs but must also be flexible to assist the team as needed. This position is part of an Interdisciplinary Team (IDT) that meets bi-weekly to discuss patient eligibility.

Requirements

  • Graduate of an approved school of nursing (AD, Diploma, or BSN program).
  • Current license to practice professional nursing in North Carolina / South Carolina.
  • Two years of experience.
  • Hand/eye coordination to give injections and use a computer.
  • Ability to speak clearly to communicate with patients, families, physicians, and staff.
  • Must be able to communicate and be literate in the English language.
  • Ability to develop positive interactions with patients, families, physicians, and staff.
  • Ability to lift and/or maneuver objects weighing at least 45 pounds.
  • Requires pushing, moving, lifting of patients.
  • Must be able to assess a patient’s condition, formulate a Plan of Care, and implement appropriate interventions.
  • Requires higher level of mental faculties accompanied by short and long-term memory.
  • Must be able to assess patients and utilize sight to implement and evaluate Plan of Care.
  • Utilize hearing to auscultate lung sounds, bowel sounds and effectively communicate.
  • Frequent exposure to blood and body fluids, infectious diseases, needle puncture wounds.
  • May be exposed to dangerous animals and traffic hazards while home visiting.
  • May encounter patients and other situations that present a potential threat to personal safety.
  • May encounter temperature changes and weather extremes.
  • Will be required to drive.
  • Must have reliable transportation.
  • Will be subject to the Safe Driver Policy.
