Requirements

Licensed Registered Nurse (RN) in Massachusetts
Compassionate, with a deep understanding of the emotional complexities involved in end-of-life care.
An exceptional communicator and a strong advocate for patient and family needs.
A team player who collaborates effectively with interdisciplinary teams.

Responsibilities

Serve as a primary coordinator of all care for hospice patients, ensuring seamless and compassionate end-of-life care.
Conduct comprehensive assessments and create personalized care plans in collaboration with patients, families, and the interdisciplinary team.
Provide expert nursing care, symptom management, and emotional support tailored to each patient's needs.
Act as a liaison, advocating for patients' and families' needs and coordinating with healthcare providers and community resources.
Educate families and caregivers on end-of-life care, ensuring they feel supported and informed.
