About The Position

Cedar Valley Hospice, a not-for-profit organization with multiple local and national awards for high-quality, exceptional end-of-life care, is seeking a compassionate nurse for the role of Hospice RN. As a Hospice RN, you will work alongside an experienced interdisciplinary team to provide expert clinical care, education, and support to patients and families in the community. Cedar Valley Hospice is the premier hospice provider serving 15 counties with offices in Waterloo, Grundy Center, Independence, and Waverly, committed to helping patients live fully and comfortably wherever they call home. Requirements

  • Current Iowa Registered Nurse (RN) license
  • Strong communication and interpersonal skills
  • Positive attitude and commitment to teamwork
  • Basic computer proficiency and ability to learn work-related software
  • Valid Iowa driver's license, reliable transportation, and auto insurance
  • Ability to move up to 50 pounds with or without reasonable accommodation
  • Ability to travel throughout the service area and work in patient homes, healthcare facilities and office settings
  • Ability to alternate between sitting, standing, walking and driving throughout the workday

Nice To Haves

  • Bachelor of Science in Nursing (BSN)
  • Two years of recent nursing experience, preferably working collaboratively within an interdisciplinary team
  • Previous hospice, home health, palliative care, or related nursing experience

Responsibilities

  • Provide nursing care to hospice patients in homes and healthcare facilities
  • Deliver compassionate end-of-life care focused on comfort, dignity and quality of life
  • Assess patients physical, emotional, psychosocial and spiritual needs
  • Collaborate with physicians and the interdisciplinary team to develop individualized care plans
  • Monitor changes in condition and update plan of care
  • Participate in interdisciplinary team meetings and care planning discussions
  • Educate and support patients, caregivers and families
  • Coordinate care and advocate for patients and families throughout their hospice journey
