About The Position

We are seeking a compassionate, detail‑oriented, and hospice‑experienced Nurse Practitioner to support our Hospice program. This role requires active PECOS enrollment, the ability to complete timely Face‑to‑Face (F2F) encounters, and strong clinical judgment to support terminally ill patients and their families. Requirements

  • Current unrestricted Nurse Practitioner in Nevada required.
  • PECOS -Enrolled for Hospice required.
  • Understanding of Medicare hospice regulations, eligibility, and documentation.
  • Strong clinical assessment and documentation skills
  • Willing and able to meet the agency need to support hospice Medical Director/designee.
  • Excellent interpersonal skills, sound judgement, effective organizational, prioritization and follow-through skills, constructive relationships with communication and collaboration at all levels required.
  • Positive, professional interpersonal skills.
  • Reliable transportation for home visits.

Nice To Haves

  • Experience in a Medicare-certified home health/hospice agency preferred.

Responsibilities

  • Performs routine and urgent assessments: Completing Medicare‑required Hospice Face‑to‑Face (F2F) visits for recertification
  • Conducting comprehensive assessments to determine continued eligibility
  • Documenting clinical findings clearly, accurately, and in compliance with CMS guidelines
  • Orders, performs, and interprets laboratory and radiology tests within scope of professional practice to assess patient’s clinical problems and health care needs.
  • Prescribes medications including controlled substances to the extent delegated and licensed.
  • Orders treatments and durable medical equipment as indicated.
  • Performs other therapeutic measures as indicated.
  • Assists in all facets of care coordination
  • Assists patients and families in identification of goals of treatment and ongoing plans of care at every visit.
  • Communicates these goals among Interdisciplinary Team.
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
  • Complies with applicable laws and regulations with respect to Collaborative Agreements.
  • Maintaining active PECOS enrollment for ordering, certifying, and billing requirements
  • Collaborates/communicates with Medical Director, attending Physician, hospital staff, and Interdisciplinary Team:
  • Participates in Interdisciplinary Team meeting bi-weekly, QA and other meetings as assigned.
  • Collaborates/communicates with Interdisciplinary Team needs of patient for clinical and psycho-social interventions.
  • Assists in identifying the need for intervention of other Interdisciplinary Team members.
  • Effectively communicates patient and family needs to Interdisciplinary Team.
  • Completes and submits required clinical documentation within 24 hours of visit completion.
  • Educates patient and caregiver regarding: Care of patient. Disease process. Goal setting. Symptom control. Treatment options. Prognosis. Advance Care Planning.
  • Supervises nurse trainees, including graduate and advanced practice nurses in field placements.
  • Must be able to operate an automobile/has access to personal transportation.
  • Able to lift/transfer/push/pull up to 100 pounds infrequently using assistive techniques and devices appropriate to manage the task.
  • Must be able to complete multi-tasks and meet multi-deadlines.
  • Proactively promotes census growth and seeks to offer care that meets the unmet diverse needs of the community.
  • Performs other duties as assigned.
