About The Position

Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks an experienced and compassionate nurse practitioner to join our dynamic team in a full-time role. The hospice nurse practitioner will join our expanding interdisciplinary team providing care for hospice patients in the community setting.

Requirements

  • Master’s degree from an accredited program and a current nurse practitioner license valid in the state of Massachusetts
  • 2 years of nursing experience with at least 1 year of experience in hospice, palliative care, or oncology
  • Familiarity with Medicare Part A hospice regulations and eligibility requirements
  • A professional and caring demeanor
  • Ability to take initiative, work independently, and manage time well
  • Outstanding communication skills and willingness to converse with other members of the hospice interdisciplinary team, medical staff in the community, and patients and their families
  • Comfort working with culturally diverse patients/families
  • Able to document in an electronic medical record, after adequate training

Responsibilities

  • Complete face-to-face visits for initial and ongoing hospice eligibility, assessing disease-related changes, declines in functional and cognitive status, and symptom burden, including effectiveness of current interventions
  • Communicate significant visit findings (including unmanaged symptoms and eligibility concerns) to the designated hospice physician, in a timely fashion
  • Engage with, and work alongside, other members of the hospice interdisciplinary team
  • Provide symptom management recommendations for current hospice patients, as necessary
  • Prescribe medications for current hospice patients, as necessary
  • Document visits in a timely fashion
