Requirements
- Current license from the state Board of Registered Nursing
- Master’s Degree from an accredited Nurse Practitioner’s Program
- National Certification as a Nurse Practitioner or certification eligible (must be obtained within the first six (6) months of employment)
- Valid driver’s license and reliable transportation
- Able to pass criminal background check and national sex offender clearance
- Must be able to perform patient care as needed
Nice To Haves
- A minimum of one (1) year Nurse Practitioner experience preferred
- Previous Hospice experience preferred
- OR and WA license
Responsibilities
- Rounding in hospitals and skilled nursing facilities.
- Reviewing patients currently on our services and updating care plans accordingly.
- Under the direction of the Medical Director, assessing patients, reviewing medications, creating appropriate care plans, and providing and evaluating care.
- Assisting with advanced directives and discussing goals of care, disease process, progression, and grief process with patients and caregivers.
- Working with the Director of Patient Care Services and Executive Director to provide education on Palliative and Hospice care within the community and to agency staff regularly.
