About The Position

We are hiring a Nurse Practitioner for a Hospice position providing end-of-life care to patients in their homes in the New Kensington, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key milestones during your first year!

Requirements

• Bachelor of Science (BSN) degree from an accredited School of Nursing
• Master of Science (MSN) degree from an accredited School of Nursing
• A minimum of five (5) years of clinical experience
• Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work

Nice To Haves

• Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred.

Responsibilities

• Provide compassionate, patient-centered hospice care, collaborating with an interdisciplinary team to enhance comfort, dignity, and quality of life for patients and families.
• Conduct comprehensive clinical assessments, manage complex symptoms and medications, and develop individualized plans of care that align with patient goals and end-of-life preferences.
• Educate and support patients, families, and caregivers through clinical decision-making and care planning while ensuring accurate, timely, and compliant EMR documentation through strong critical thinking and problem-solving skills.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
