The Care Team Hospice is looking for a full-time Nurse Practitioner to see patients in the home and in the facility. This role offers a new and exciting opportunity to join a growing team. The Nurse Practitioner will be responsible for the care of terminally ill patients, operating in a systematic manner to manage the healthcare needs of these patients. Care provided will be tailored to the cultural, religious, and social needs of the dying patient. The position involves regular assessment and reassessment of patient needs, timely face-to-face visits, dietary counseling, providing nursing services, treatments, and preventive procedures. The Nurse Practitioner will observe and report changes in patient condition, teach and counsel patients and families, supervise and train other nursing personnel, and develop/re-evaluate patient/family care plans. Specific nursing procedures will be performed as needed, following physician orders. The role requires attendance at team conferences, maintenance of required records, adherence to Hospice policies and procedures, and compliance with HIPAA regulations. Communication with supervisors regarding patient needs or procedural limitations is essential. Participation in the on-call system is required, as is maintaining skills and knowledge. The Nurse Practitioner will work within the interdisciplinary group concept of patient care, coordinate care for patients in various facility types, and organize their work schedule for efficient time management and meeting attendance. Compliance with agency infection control policies and protocols is mandatory, as is assisting with orientation, teaching, and training as requested. Clinical notes for each service provided, including each home visit, must be completed within 48 hours. Other duties may be assigned by the Director.
