About The Position

The Nurse Practitioner - PPV at Compassus is responsible for embodying the company's core values of Compassion, Integrity, Excellence, Teamwork, and Innovation, and for promoting its philosophy based on the 6 Pillars of Success. The Nurse Practitioner provides care for patients facing chronic, complex, and/or life-threatening conditions. If providing palliative care, the NP conducts assessments, offers disease and treatment education, and manages symptoms for patients and their families across various locations, collaborating with primary care physicians and specialists. If providing hospice care, the NP acts as an extension of the interdisciplinary team and hospice physician, performing routine recertification and emergency assessments, educational, and evaluative services. In both capacities, the goal is to help patients and families maintain maximum comfort and coping. The role demands compliance with practice standards, collaboration with the assigned Compassus Medical Director, and a flexible schedule to meet agency needs, potentially including after-hours emergency visits or admissions.

Requirements

  • Master’s degree in Nursing
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
  • Active NPI number
  • Maintain active DEA certification if state allows prescription of controlled substances by APPs
  • Minimum of one (1) year nursing practice or equivalent experience (if providing Hospice Care)
  • Minimum of five (5) years nursing experience (if providing Palliative Care)
  • Current collaborative physician agreement in place (All states except Iowa)

Nice To Haves

  • Hospice, oncology, or home health and/or long-term care experience (if providing Hospice Care)
  • Clinical nursing care in oncology, hospice, geriatrics, intensive care (if providing Palliative Care)
  • Board Certification in Hospice and Palliative Care

  • Helping the patient and family maintain their maximum level of comfort and coping
  • Providing care and support in collaboration with the PCP and specialists (if providing palliative care)
  • Ensuring compliance with practice standards and codes (if providing palliative care)
  • Collaborating appropriately with the assigned Compassus Medical Director
  • Flexing schedule to meet agency needs
  • Functioning as an extension of the interdisciplinary team (IDT) and hospice physician to provide routine recertification and emergency assessments, educational, and evaluative services (if providing hospice care)
  • Completing routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond (including admissions and transfers) (if providing Hospice Care or Hospice Face to Face Visits)
  • Completing routine and emergency assessments on each patient as indicated by departmental policy (if providing Hospice Care)
  • Recording observations, treatments, and other pertinent information
  • Communicating with Medical Director, Attending Physician, DCS and IDT as directed regarding findings from the visits and any measures to alleviate symptoms
  • Providing physical and emotional care to support the patient and family in period of crisis
  • Preventing unwarranted hospitalizations by meeting patient and family needs in the home (if providing Hospice Care)
  • Performing routine and emergency assessments related to outpatient palliative care visits (if providing palliative care)
  • Performing bedside palliative care patient/family evaluation in the patient’s place of residence (if providing palliative care)
  • Assisting with diagnosis, treatment, and management of acute and chronic health conditions (if providing palliative care)
  • Ordering, performing, and interpreting laboratory and radiology tests within scope of professional practice (if providing palliative care)
  • Prescribing medications including controlled substances to the extent delegated and licensed (if providing palliative care)
  • Ordering treatments and durable medical equipment as indicated (if providing palliative care)
  • Performing other therapeutic or corrective measures as indicated, including urgent care (if providing palliative care)
  • Consulting with palliative care physician or designees as needed, informs primary physician of services provided and collaborates with other physicians as needed (if providing palliative care)
  • Assisting in all facets of care coordination for palliative care referrals (if providing palliative care)
  • Preparing and maintaining accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided (if providing palliative care)
  • Complying with applicable laws and regulations with respect to Collaborative Agreements (if providing palliative care)
  • Initiating reimbursement for services rendered (if providing palliative care)
  • Assisting patients and families with goal setting related to treatment options and advanced care planning (if providing palliative care)
  • Providing training and continuing education for staff (if providing palliative care)
  • Assisting in development of clinical practice guidelines/standards in support of quality palliative care (if providing palliative care)
  • Adhering to the practice of confidentiality regarding patients, families, staff and the organization (if providing palliative care)
  • Collaborating/Communicating with IDT needs of patient for clinical and psycho-social interventions
  • Assisting in identifying the need for intervention of other IDT members
  • Effectively communicating patient and family needs to IDT
  • Completing and submitting required paper work within 24 hours of visit completion
  • Educating patient and caregiver regarding care of patient, disease process, dying process, symptom control, and wound care
  • Performing other duties as assigned
