About The Position

The Hospice Nurse Liaison is responsible for evaluating and admitting patients to the GIP, routine hospice service, and developing appropriate plans of care, completes required assessments, and develops the initial plan of care while maintaining timely and accurate documentation. The Hospice Nurse Liaison is responsible for the development and maintenance of referral relationships and assists referral sources with smooth and effective referrals to the agency.

Requirements

  • Associates Degree of Nursing required
  • Knowledge of State Regulations, Medicare, and Medicaid regulations, and general inpatient hospice, PDGM/ Home Health/ Hospice is required.
  • Experience in home health or acute care setting.
  • Current Connecticut Registered Nurse License

  • Healthcare business development and marketing experience preferred

Responsibilities

  • Provide assessment for appropriateness of Hospice services across assigned settings.
  • Meets with patients and family members to provide education about Hospice and sign consents for admission.
  • Responsible for identifying the appropriate payer for services in keeping with payer qualifications and conditions, and for providing appropriate notification in situations of non-coverage.
  • Complete Hospice admissions across assigned settings including GIP and Routine Hospice admissions while ensuring patients meet required criteria for admission, including completing required assessments including HOPE and the comprehensive assessment, and completing documentation accurately and timely.
  • The Hospice Nurse Liaison provides timely and appropriate referrals and admissions to hospice services including assistance with the ordering of DME.
  • Responsible for utilization and integration of best practice standards in the assessment of patients and families and in development of patient-specific care plans.
  • Responsible for the reconciliation of medications, initial medication assessment and teaching and establishment of the medication profile.
  • Completes hospice re-visits for GIP and assigned routine visits.
  • Case management of assigned patient caseload.
  • Responsible for discharge planning of GIP hospice patients to alternate settings as applicable.
  • Serves as a resource to referral contacts including physicians, palliative care team, case managers, and social workers. Develops and nurtures referral relationships.
  • Always represents Masonicare positively and professionally.
  • Assists with service recovery and education to both patients, families, and referral sources with an understanding of hospice philosophy, the hospice Medicare Benefit, services, and transitions of care.
  • Responds to referrals for hospice services timely.
  • Assist hospital case managers with referrals to the agency for Routine and GIP hospice. Coordinates with Clinical Liaisons for all other referrals.
  • Collaboration with Clinical Liaisons regarding referrals and business development opportunities.
  • Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
  • Other duties as assigned.
