About The Position

This role involves providing skilled nursing care to patients at the end of life (EOL), conducting visits in both facility and home settings. The Hospice Nurse will offer on-call services after hours and complete scheduled skilled visits. Key responsibilities include responding to patient and family calls regarding concerns about symptoms, changes in conditions, or emergencies, and conducting home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis. The nurse will also assist the RN Case Manager (RNCM) in executing the plan of care, performing specialized procedures as directed, and reinforcing patient and family education regarding end-of-life care. The position encourages personal and vocational growth through active participation in nursing organizations, in-service programs, and continuing education. The nurse will keep the RNCM informed of any changes noted during patient care and assist with emergent visits, including continuous care (CC) and companion care as needed. Other duties as assigned.

Requirements

  • Licensed RN in the state of practice
  • A minimum of 1 year of Clinical experience
  • Valid Driver's License and Auto Insurance Coverage
  • Reliable transportation, willingness to travel
  • On-call/ skilled nursing experience required

Responsibilities

  • Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
  • Provide on-call services after hours, and complete scheduled skilled visits.
  • Respond to phone calls from patients and their families regarding concerns about symptoms, changes in conditions, or emergencies.
  • Conduct home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis.
  • Assist RNCM in carrying out the plan of care.
  • Assist the Nursing team in performing specialized procedures as defined in the state nurse practice act as directed by the RNCM under the physician's orders.
  • Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
  • Assist in the reinforcement of patient and family education regarding end-of-life care.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
  • Keep the RNCM informed of changes noted during the process.
  • Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading.
  • Assist with emergent visits including continuous care (CC) and companion care as needed.
  • All other duties as assigned
