About The Position

Residential Home Health and Hospice is seeking a Medical Social Worker (MSW) for a Hospice position. This role involves providing care to patients in their homes, focusing on comfort, dignity, and quality of life during the end-of-life journey. The position offers autonomy, meaningful patient relationships, and career growth opportunities within a nationally recognized leader in home health and hospice.

Requirements

• Master’s in Social Work from graduate school accredited by the Council of Social Work.
• One (1) to two (2) years post graduate social work experience in a health care setting.
• A valid driver’s license.
• Ability to spend approximately 20% of your workday driving to and from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team.
• Conduct comprehensive psychosocial and bereavement assessments, identifying patient and family needs.
• Develop care plans that support emotional, social, and end-of-life goals.
• Provide counseling, emotional support, and guidance to patients and families.
• Connect patients and families with resources.
• Navigate the end-of-life journey with patients and families.
• Apply critical thinking and compassionate problem-solving to address complex situations.
• Advocate for patient wishes.
• Ensure accurate, timely, and compliant EMR documentation.
• Travel to patient homes within assigned territory.
