About The Position

We are hiring a Medical Social Worker (MSW) for a Hospice position providing care to patients in their homes in the Jefferson Hills, PA area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The MSW will conduct comprehensive psychosocial and bereavement assessments, identify patient and family needs, and develop care plans that support emotional, social, and end-of-life goals. They will provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey. This role requires critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation. The position involves working independently, traveling to patient homes within an assigned territory, and being supported by a dedicated field education team.

Requirements

  • Master’s in Social Work from graduate school accredited by the Council of Social Work
  • One (1) to two (2) years post graduate social work experience in a health care setting
  • Current driver’s license
  • Ability to spend ~20% of your day driving to/from patient locations
  • Legally authorized to work in the United States
