About The Position

Optimal Care is proactively building a talent pipeline for future opportunities as a Hospice Medical Social Worker. This role involves providing compassionate, clinical psychosocial care to help patients and families navigate the emotional, social, and practical challenges of end-of-life illness. The Hospice Medical Social Worker will support individuals facing terminal diagnoses by offering counseling, guidance, and resources to promote dignity, clarity, and emotional well-being. This position requires a licensed clinical social worker passionate about practicing the full scope of hospice social work. Responsibilities include completing comprehensive psychosocial assessments, providing crisis intervention and counseling, and supporting advance care planning conversations. The role involves close collaboration with the interdisciplinary team (nurses, physicians, hospice professionals) to ensure emotional, cultural, and practical needs are integrated into the patient's plan of care, thereby supporting both patients and families and strengthening the overall hospice journey.

Requirements

  • Master's or doctoral degree from a CSWE-accredited school of social work
  • Current state licensure as a Licensed Master's Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
  • Minimum 1 year of experience as a licensed Master's Social Worker
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
  • Willingness to travel to homes, facilities, and community settings

Nice To Haves

  • Hospice or palliative care experience
  • Clinical experience with grief counseling and end-of-life issues
  • Knowledge of Medicare hospice benefit and Conditions of Participation

Responsibilities

  • Provide compassionate, clinical psychosocial care to patients and families navigating end-of-life illness.
  • Offer counseling, guidance, and resources to promote dignity, clarity, and emotional well-being.
  • Complete comprehensive psychosocial assessments.
  • Provide crisis intervention and counseling.
  • Support advance care planning conversations.
  • Collaborate with nurses, physicians, and other hospice professionals as part of the interdisciplinary team.
  • Ensure emotional, cultural, and practical needs are integrated into the patient's plan of care.
