About The Position

Optimal Care is proactively building a talent pipeline for future opportunities as a Hospice Medical Social Worker. This role involves providing compassionate, clinical psychosocial care to help patients and families navigate the emotional, social, and practical challenges of end-of-life illness. As a Hospice Medical Social Worker, you will support individuals facing terminal diagnoses by offering counseling, guidance, and resources that promote dignity, clarity, and emotional well-being. This role is ideal for a licensed clinical social worker passionate about practicing the full scope of hospice social work. You will complete comprehensive psychosocial assessments, provide crisis intervention and counseling, and support advance care planning conversations. Working as a key member of the interdisciplinary team, you will collaborate closely with nurses, physicians, and other hospice professionals to ensure emotional, cultural, and practical needs are integrated into each patient’s plan of care. Your work will directly support both patients and families while strengthening the care experience across the entire hospice journey.

Requirements

  • Master's or doctoral degree from a CSWE-accredited school of social work
  • Current state licensure as a Licensed Master's Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
  • Minimum 1 year of experience as a licensed Master's Social Worker
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
  • Willingness to travel to homes, facilities, and community settings

Nice To Haves

  • Hospice or palliative care experience
  • Clinical experience with grief counseling and end-of-life issues
  • Knowledge of Medicare hospice benefit and Conditions of Participation

Responsibilities

  • Provide compassionate, clinical psychosocial care to patients and families navigating end-of-life illness.
