About The Position

Luna Del Valle Hospice is seeking a Part-Time Hospice Social Worker (MSW) to serve patients in the Gallup area. This role is crucial for supporting patients and families through emotional, social, and financial transitions associated with end-of-life care. The Social Worker will collaborate with an interdisciplinary team to ensure compassionate care that addresses the full spectrum of patient needs. The position involves responding to referrals, performing in-depth assessments, offering counseling, assisting with resource navigation, planning, and connection to community services. Accurate record-keeping, participation in case discussions, home visits, and on-call duties are also part of the role. The ideal candidate will have a Master's in Social Work, at least one year of social work experience (hospice or healthcare preferred), and be licensed to practice Social Work in New Mexico, with current CPR certification and a valid driver's license.

Requirements

  • Master’s in Social Work (MSW) from a CSWE-accredited program
  • Minimum of one year in a social work role
  • Licensed to practice Social Work in New Mexico
  • Current CPR certification
  • Valid driver’s license and reliable transportation with auto insurance
  • Excellent communication and time management skills
  • Deep empathy and a calm, professional demeanor
  • Familiarity with Medicare/Medicaid and community resource networks
  • Ability to work independently in home-based environments

Responsibilities

  • Respond to referrals from the clinical team for social work support
  • Perform in-depth assessments to identify emotional, social, and economic concerns
  • Offer brief counseling and assist families with navigating available resources
  • Assist with planning, goal-setting, and connection to community-based services
  • Keep accurate records and maintain documentation in accordance with care plans
  • Participate in case discussions and contribute to interdisciplinary planning
  • Provide home visits and be available for on-call duties as required
  • Maintain confidentiality while preserving patient dignity throughout care
