About The Position

We are currently seeking a licensed and experienced Medical Social Worker to join our team. As a Medical Social Worker at Luna Del Valle Hospice, you will play a vital role in providing emotional support, counseling, and resources to our patients and their families during their end-of-life journey. We are looking for candidates who are licensed Social Workers with experience in end-of-life care and a compassionate and empathetic approach to patient care.

Requirements

  • Current licensure as a Medical Social Worker.
  • Experience in end-of-life care or hospice settings.
  • Strong communication and interpersonal skills.
  • Compassionate and empathetic approach to patient care.
  • Ability to work effectively as part of a multidisciplinary team.
  • Social Worker License/Certification (Required)

Responsibilities

  • Provide comprehensive psychosocial assessments of patients and their families to identify their needs and concerns.
  • Offer supportive counseling and guidance to patients and families to help them cope with the emotional and psychological aspects of end-of-life care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the social, emotional, and spiritual needs of patients.
  • Connect patients and families with community resources and support services as needed.
  • Maintain accurate and timely documentation of all patient interactions and interventions.
