We are currently seeking a licensed and experienced Medical Social Worker to join our team. As a Medical Social Worker at Luna Del Valle Hospice, you will play a vital role in providing emotional support, counseling, and resources to our patients and their families during their end-of-life journey. We are looking for candidates who are licensed Social Workers with experience in end-of-life care and a compassionate and empathetic approach to patient care.

Requirements

Current licensure as a Medical Social Worker.
Experience in end-of-life care or hospice settings.
Strong communication and interpersonal skills.
Compassionate and empathetic approach to patient care.
Ability to work effectively as part of a multidisciplinary team.
Social Worker License/Certification (Required)

Responsibilities

Provide comprehensive psychosocial assessments of patients and their families to identify their needs and concerns.
Offer supportive counseling and guidance to patients and families to help them cope with the emotional and psychological aspects of end-of-life care.
Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the social, emotional, and spiritual needs of patients.
Connect patients and families with community resources and support services as needed.
Maintain accurate and timely documentation of all patient interactions and interventions.
