About The Position

We are hiring a Medical Social Worker (MSW) for a Hospice position providing care to patients in their homes in the Ann Arbor, MI area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The MSW will conduct comprehensive psychosocial and bereavement assessments, develop care plans, provide counseling and emotional support, and connect patients and families with resources. Critical thinking and compassionate problem-solving are essential for addressing complex situations and advocating for patient wishes. Accurate and timely EMR documentation is also a key responsibility. The role requires independent work within an assigned territory, with travel to patient homes.

Requirements

  • Master’s in Social Work from graduate school accredited by the Council of Social Work.
  • One (1) to two (2) years post graduate social work experience in a health care setting.
  • Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work.
  • A valid driver’s license.
  • Ability to spend approximately 20% of your workday driving to and from patient locations are required.

Responsibilities

  • Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
  • Conduct comprehensive psychosocial and bereavement assessments, identifying patient and family needs while developing care plans that support emotional, social, and end-of-life goals.
  • Provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey with clarity and dignity.
  • Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation.
  • Work independently but never alone.
  • Travel to patient homes within assigned territory.
