About The Position

Suncrest At Brighton/Suncrest Hospice is a CHAP-certified hospice provider focused on changing the expectation of hospice care through exceptional patient care and investment in its people. The Hospice Social Worker provides compassionate psychosocial support to patients, families, referring sources, and team members navigating the challenges of serious illness and end-of-life care. This role involves acting as an advocate, resource, and trusted member of the interdisciplinary hospice team to support patients and families throughout their hospice journey.

Requirements

• Social Work degree that complies with CHAP and state of residence regulations.

Responsibilities

• Assess patients’ and families’ psychosocial, emotional, spiritual, and socioeconomic needs and incorporate findings into the individualized plan of care.
• Counsel patients and families while providing emotional support, crisis intervention, and guidance as they navigate illness, loss, grief, and significant life transitions.
• Educate patients and families about available community resources and connect them with appropriate programs, services, and support.
• Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs, preferences, values, and goals are heard and addressed.
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other members of the care team.
• Maintain accurate, thorough, and timely documentation in accordance with hospice standards and regulatory requirements.
