This role supports our Hand in Hand hospice program, a part of Arc Hospice & Palliative Care family, and is dedicated to compassionate, community-rooted end-of-life care. This is a PRN (as needed) position supporting hospice patients across our service area. Assignments may vary based on patient needs and coverage requirements.

Requirements

  • Current LPN or LVN license in good standing in the state of practice or a valid compact license covering all states of operation.
  • Minimum of one (1) year of clinical nursing experience preferred, ideally in hospice, home health, or geriatrics.
  • Demonstrated understanding of hospice philosophy, symptom management, and end-of-life care practices.
  • Proficiency in electronic medical record (EMR) documentation and mobile clinical tools.
  • CPR certification required.
  • Valid driver’s license, reliable transportation, and proof of automobile insurance.
  • Strong communication skills and the ability to function effectively as part of an interdisciplinary care team.
  • Must demonstrate a strong commitment to Arc Hospice & Palliative Care’s mission and values.

Responsibilities

  • Provide direct nursing care to hospice patients under the supervision of a Registered Nurse, in alignment with the plan of care and scope of practice.
  • Perform scheduled and unscheduled nursing visits, including comfort care, medication administration, and skilled observation.
  • Document all care and communication accurately and promptly in the electronic medical record (EMR).
  • Support pain and symptom management interventions, reporting changes in condition to the supervising RN or physician as appropriate.
  • Collaborate with interdisciplinary team members to ensure coordinated, patient- and family-centered care.
  • Educate patients and families on care techniques, medications, and symptom awareness as directed.
  • Provide support for emergency visits, tuck-in calls, respite, and Continuous Care as appropriate.
  • Participate in on-call rotations and respond to after-hours clinical needs when scheduled.
  • Maintain compliance with agency policies, hospice regulations, and professional standards of practice.
  • Performs other duties as required.
