The Hospice LVN Intake Supervisor will be responsible for oversight of the intake process to accept and process referrals to By the Bay Health Services. This includes informational interviews with patients and families, review of eligibility for the Hospice benefit, physician orders, patient assessment, and processing of regulatory and organizational documents. The supervisor is responsible for achieving organizational metrics related to the intake team and processes. The position will foster collaboration between intake, admission nurses, and provider relations to allow for seamless processes and positive patient and family experiences. The position is expected to build and maintain optimal relationships with all referral sources in compliance with organization policies and procedures and applicable laws and regulations. This role serves as Intake Supervisor in accordance with BTBH organizational policies and procedures Schedule: Monday - Friday; 8:30am - 5:00pm Location: Larkspur, CA By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Requirements

  • Minimum of two years of supervisory experience in a home health/ hospice setting within the past five years.
  • Five years experience as a Licensed Vocational Nurse (LVN).
  • Current license as a Licensed Vocational Nurse (LVN) in the State of California required.

Responsibilities

  • Ordering medical supplies and medications (bipap, tube feeding, CADD pumps, comfort meds, etc.)
  • Screen and process inquiries for admission to By the Bay Health’s programs of service.
  • Determine eligibility in consultation with Hospice Medical Director.
  • Assume responsibility for assessing initial patient/family needs.
  • Supervises non-exempt clinical and non-clinical Intake Department
