The Hospice Intake Supervisor will be responsible for oversight of the intake process to accept and process referrals to By the Bay Health Services. This includes informational interviews with patients and families, review of eligibility for the Hospice benefit, physician orders, patient assessment, and processing of regulatory and organizational documents. The supervisor is responsible for achieving organizational metrics related to the intake team and processes. The position will foster collaboration between intake, admission nurses, and provider relations to allow for seamless processes and positive patient and family experiences. The position is expected to build and maintain optimal relationships with all referral sources in compliance with organization policies and procedures and applicable laws and regulations. This role serves as Intake Supervisor in accordance with BTBH organizational policies and procedures By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Requirements

  • Minimum of two years of supervisory experience in a home health/ hospice setting within the past five years.

Responsibilities

  • Provide direct supervision to both clinical and administrative staff to support the admission processes.
  • Ensure review of all inquiries for intake to organization’s total care category.
  • Plan for informational and admission services to patients as needed.
  • Implement and monitor compliance with organization policies and procedures related to clinical services.
  • Oversee supervision of intake personnel, completing written competencies, performance evaluations and skills checklist in accordance with organization policy, accreditation, and regulatory standards.
  • Supervises non-exempt clinical and non-clinical Intake Department
