About The Position

The Hospice LVN Intake Coordinator is to facilitate the referral intake process for potential hospice patients, including receiving referrals, verifying benefits, gathering necessary medical information, and coordinating care with the Interdisciplinary Group/Team (IDG/IDT), the patient, their family, and the referral source for a smooth transition into hospice care.

Requirements

  • Graduate of an accredited school of Vocational/Practical nursing.
  • Current state license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC).
  • Current driver’s license.
  • Minimum of 2 years of experience in healthcare intake or admissions.
  • Proficiency with EMR systems and Microsoft Office.
  • Strong organizational and communication skills.
  • Ability to manage multiple tasks and prioritize effectively.
  • Knowledge of Medicare and Medicaid regulations.
  • Clinical assessment and documentation skills.

Responsibilities

  • Promotes the Hospice’s philosophy and mission by presenting a positive image to patients and families, physicians and community agencies.
  • Uses effective customer service skills when directing calls and assisting others.
  • Answers incoming calls from potential patients, healthcare providers, and other referral sources to assess eligibility for hospice services.
  • Receives and appropriately documents new referrals in a timely manner and completes the benefit verification process.
  • Gathers medical information/records and other pertinent paperwork from referral sources to maintain comprehensive intake files.
  • Participates in coordinating care with Nursing Supervisors, patients, and the Interdisciplinary Group/Team to ensure seamless patient care transition.
  • Provides effective communication to patients, staff members, other health care professionals, and referral sources to foster and maintain positive relationships.
  • Responds to the Hospice’s and patient needs in a professional and creative manner to address any concerns and provide support.
  • Demonstrates commitment, professional growth, and competency.
  • Participates in the Hospice’s sponsored in-service trainings.
  • Participates in the Hospice’s QAPI program.
  • Performs other duties as required.
