Hospice LPN PRN
About The Position
Elara Caring is seeking a Licensed Vocational/Practical Nurse to provide compassionate, high-quality hospice care in patients’ homes. The nurse follows individualized care plans and physician orders, monitors patient progress, communicates changes and safety concerns to the care team, and supports patients and caregivers with self-care techniques. Working with the RN Case Manager, the nurse helps keep care plans and medication profiles current and coordinates care with physicians, patients, families, and other team members. The role also focuses on proactive care to help prevent avoidable hospitalizations and emergency room visits.
Requirements
- Current state license as a Licensed Vocational/Practical Nurse.
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
- One year of experience in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

- Work with the RN Case Manager to update care plans as patient needs change.
- Document medication regimens and update medication profiles according to agency guidelines.
- Communicate with physicians, patients, families, and the care team to ensure coordinated care.
- Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
