About The Position

We are hiring a Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Wexford, PA area and is eligible for a $5,000 sign-on bonus! The role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. Responsibilities include conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, as well as educating and guiding patients and caregivers on end-of-life care, symptom relief strategies, and what to expect throughout the hospice journey. The position requires thorough, compliant EMR documentation and the ability to work independently while traveling to patient homes within an assigned territory.

Requirements

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities
