About The Position

RVNAhealth is seeking a Full Time Hospice Licensed Practical Nurse (LPN) to join our dedicated hospice team. In this field-based role, you'll provide direct patient care under the supervision of a Registered Nurse, delivering both intermittent and continuous care visits in accordance with personalized care plans. You'll be part of a collaborative, interdisciplinary team that supports patients and families with compassion, dignity, and professionalism. This is a full-time position (Monday-Friday, 8:30am - 4:30pm) and will cover the area of Southern Fairfield County, including Stamford and Greenwich.

Requirements

  • Graduate of an accredited nursing program.
  • Active LPN license in the State of Connecticut.
  • Minimum 2 years of clinical experience (MA, CNA, or similar role).
  • Strong understanding of community-based care and hospice regulations.
  • Ability to manage multiple tasks and maintain professionalism under pressure.
  • Comfortable operating a motor vehicle in all weather conditions.
  • Computer-literate with proficiency in electronic documentation systems.

Nice To Haves

  • At least 1 year of hospice or home care experience.

Responsibilities

  • Deliver direct nursing care in accordance with physician orders and under RN supervision.
  • Monitor and report patient status and changes to RN Case Manager and clinical leadership.
  • Assist with updates to the nursing care plan based on patient needs.
  • Participate in routine Interdisciplinary Group (IDG) meetings to ensure coordinated care.
  • Provide emotional support and education to patients and families.
  • Participate in the hospice's continuous care rotation as scheduled.
  • Maintain compliance with Medicare, Medicaid (Title 19), and insurance documentation requirements.
  • Use nursing equipment appropriately and maintain accurate electronic documentation.
  • Support and back up other hospice team roles when needed.
  • Engage in ongoing quality improvement initiatives and in-service training.
