Requirements

  • Graduation from an accredited Licensed Practical Nurse program.
  • Current LPN license in the state of practice.
  • Current Driver’s License.
  • Minimum of one year of nursing experience, preferably in hospice or palliative care.
  • Nursing skills as defined as generally accepted standards of practice.
  • Knowledge of state and federal regulations.
  • Ability to work independently and make sound decisions.
  • Strong attention to detail and organizational habits.
  • Excellent verbal and written communication skills.
  • Good interpersonal skills.
  • Reliable transportation and valid auto liability insurance.

Responsibilities

  • Provide direct patient care to hospice patients in their homes or hospice facilities.
  • Conduct comprehensive assessments of patient needs, including physical, emotional, and spiritual aspects.
  • Administer medications, treatments, and interventions as prescribed by the hospice interdisciplinary team.
  • Monitor patient vital signs, symptoms, and overall condition, reporting any changes or concerns promptly.
  • Communicate effectively with the hospice interdisciplinary team, including physicians, registered nurses, social workers, and chaplains.
  • Offer emotional support and comfort to patients and families, fostering a caring and compassionate environment.
  • Document all patient care activities accurately and thoroughly in the electronic medical record.
  • Provide education and guidance to patients and families regarding symptom management, medication administration, and end-of-life care.
  • Collaborate with other healthcare providers and community resources to ensure continuity of care for hospice patients.
  • Participate in ongoing education and training to maintain licensure and enhance clinical skills.
