Requirements
- Current Arizona LPN/LVN license
- Current BLS certification
- Valid driver’s license, reliable vehicle, and auto insurance
- Arizona Level One Fingerprint Clearance Card (or ability to obtain)
- Strong communication, organization, and critical thinking skills
Nice To Haves
- Hospice or home health experience
- Ability to manage emotional stress with compassion and professionalism
Responsibilities
- Assess and address the needs of hospice patients and their families
- Provide nursing care in accordance with physician orders and individualized care plans
- Collaborate closely with Nurse Case Managers and the interdisciplinary team
- Maintain clear, timely communication with physicians, hospice staff, and external providers
- Monitor and report on patients’ physical, emotional, social, and spiritual well-being
