Haven Home Health & Hospice is seeking a compassionate and dedicated Full-Time Hospice Licensed Practical Nurse (LPN) to join our growing hospice team. Requirements
- Current Oklahoma Licensed Practical Nurse (LPN) license
- Strong clinical assessment and communication skills
- Ability to work independently while collaborating effectively with the care team
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the service area
Nice To Haves
- Hospice, home health, or related nursing experience preferred
Responsibilities
- Providing skilled nursing care to hospice patients in their homes and care facilities
- Monitoring and reporting changes in patient condition
- Assisting with symptom management and patient comfort
- Educating patients, families, and caregivers regarding care needs
- Collaborating with the interdisciplinary hospice team
- Maintaining accurate and timely clinical documentation
- Participating in on-call, holiday, and weekend rotations as assigned
- Supporting patients and families with professionalism, empathy, and respect
