About The Position

The Care Team Hospice is looking for a part-time LPN in Lafayette, IN, and the surrounding areas. This LPN position will also conduct CNA/aide visits and has the potential to grow into a full-time opportunity. We specialize in providing hospice in the home and facilities. Requirements

Should have a minimum of one (1) year of work experience as an LPN preferable in-home care or hospital setting.
Licensed LPN in the State of Michigan.
Knowledge of general nursing practice.
Must pass a criminal background check & MVR check.
Completed health statements acknowledging the ability to perform the duties of the position.
Valid state driver's license.
Proof of current automobile insurance.
Current CPR card and TB testing per agency policy.
Must be computer literate and able to maintain simple records in English.
Graduate of an accredited school of practical nursing.
Must hold a license to practice practical nursing in Indiana
Should have a minimum of one (1) year of work experience as an LPN preferable in a Hospice setting.
Knowledge of general nursing practice.

Responsibilities

Provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse.
Provides services in accordance with each patient's home health plan of care and the LPNs' scope of practice.
Maintains compliance with federal and state regulations, reimbursement guidelines, and agency policies and procedures.
Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
Demonstrates ability to continually evaluate the patients plan of care and communicate findings with Registered Nurse.
Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physicians orders.
Evaluates the significance of assessment findings and communicates any pertinent information about the patients clinical status and ongoing needs to others in a timely fashion.
Demonstrates the ability to adequately assess and reassess pain.
Utilizes appropriate pain management techniques.
Educates the patient and family regarding pain management.
Demonstrates knowledge of medications and their correct administration based on the age of the patient and the patient's clinical assessment.
Demonstrates knowledge of medications and their correct administration, follows the seven (7) medication rights to reduce the potential for medication errors. (Right: patient; medication; dose; route; time; documentation; reason)
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Functions as a team member demonstrating collaboration with, and responsibility to, the patient, physician, and interdisciplinary team.
Performs patient care responsibilities considering the needs to the standard of care for patients of all ages.
Documents wound measurements with each comprehensive assessment and weekly thereafter.
Performs all aspects of patient care in an environment that optimizes patient safety, quality, and reduces the likelihood of medical/health care errors.
Help optimize the capital structure and support strategic initiatives.
One must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
Ensures compliance with agency policies and procedures for operations/processes, including, but not limited to, patient care, patient complaints, incidents, safety, and emergency management.
Ensures compliance with policies and procedures for infection prevention and control, standard precautions, and infection identification reporting.
Always maintains patient confidentiality in accordance with all HIPAA regulations.
The clinical record shall contain notes for each service provided.
A clinical note will be completed for each visit made to a patient's home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
Promotes patient/family education using various verbal and written communication techniques that take into account the patients/familys cultural, ethnic, and/or personal needs or preferences.
Collaborates with the patient/family to learn appropriate self-care techniques.
Submits documentation on time per agency policy.
Knowledgeable of current federal, state, and agency regulations concerning patient care, includes incident management and reporting.
Maintains regulatory requirements. Including complying with Legal Authority: Act 519, Public Acts of 1982; MCL 400.11, which states: All persons employed, licensed, registered, or certified to provide health care, must report a suspicion or reasonable cause to believe that physical, mental, or sexual abuse, neglect, or exploitation of an adult has occurred.
Participates in agencys other mandated activities, i.e., in-services, Quality Assurance, and Performance Improvement (QAPI); staff meetings.
