Requirements

Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the applicable state.
Minimum of three (3) years of nursing experience required.
Understanding of hospice philosophy and the unique needs of terminally ill patients and their families.
Ability to work flexible hours as needed.
Commitment to maintaining proficiency with information systems and applicable software programs.

Responsibilities

Provide specialized nursing care related to end-of-life and hospice services.
Collaborate with hospice aides, occupational therapists, physical therapists, social workers, speech therapists, dietitians, and clinical supervisors to support patient care.
Use, maintain, and prepare equipment and supplies while following appropriate aseptic techniques.
Provide effective pain and symptom management interventions.
Monitor and evaluate patient responses to treatments and medications.
Participate in professional development and continuing education opportunities.
Support quality assessment and performance improvement initiatives within the organization.
Perform delegated duties as assigned by the Registered Nurse.
Complete additional duties and responsibilities as assigned.
