About The Position

Queen City Hospice is a patient-centered hospice serving communities in the Cincinnati, Ohio area. The organization emphasizes local operations, bedside resources, manageable caseloads, and giving employees the tools to provide quality hospice care. The Hospice LPN provides direct patient care, follows the plan of care, supports patients and families through education, and coordinates services with the interdisciplinary group.

Requirements

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience.
• Currently licensed as an LPN/LVN in the practicing state.
• Current CPR certification.
• Good verbal and written communication and organization skills.
• Valid driver’s license and an automobile insured in accordance with state requirements.
• Understanding of the hospice philosophy and the needs of terminally ill patients.

Nice To Haves

• Community health, hospice, or medical/surgical experience is preferred.

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act.
• Implement current nursing practice based on a comprehensive assessment and the plan of care.
• Document patient services accurately and promptly to reflect the plan of care.
• Assess and provide patients and families or caregivers with education and information relevant to the diagnosis and self-plan of care.
• Participate in coordinating hospice services and report identified needs to the interdisciplinary group.
• Provide appropriate pain and symptom management.
• Evaluate patients’ responses to treatments and medications.
